As our patient, you will consult directly with Dr Jaber, where, not only will you receive all the answers to your questions, but also gives us the opportunity to obtain all information from you, and your other specialists, that is necessary to provide you with optimal treatment.
In order for Dr Jaber to provide you with an accurate assessment, You will be required to complete an intake, and submit to us a full health history report from your other specialists. Prior to your consultation, you will receive an email from us with a link to fill out this intake.
This intake must be filled out in full and in a timely manner so as to give Dr Jaber ample time to prepare for your consultation and speak with your other specialists, if applicable and necessary.
This intake also provides you, the patient, with helpful information about what to expect and how to prepare for your treatment.
We would like to take this moment to remind you that no piece of information is too small or unimportant. In fact even the smallest piece of medical information is vital to the effect and safety of your treatment.
Following the consultation, our staff will contact you to ensure that all the appropriate financial documents and processes are completed prior to your treatment.
A few days before your scheduled appointment, you will receive your Day of Appointment packet to give you all the information you will need for your day of treatment.
Following pre-operative instructions, specifically your fasting protocol, is critical to your safety. If your fasting protocol is not followed, this can result in a cancellation of your appointment and the need to reschedule.
